Strengthening Ambulatory Care for Non-Communicable Diseases (NCDs) in India (STAR-NCD)

ICMR’s National Health Research Priority Project The STAR-NCD is an implementation research initiative to enhance ambulatory care services for common NCDs such as hypertension, diabetes, chronic obstructive pulmonary disease (COPD), and secondary prevention of coronary artery disease. Implemented within the National Programme for Non-Communicable Diseases (NP-NCD) framework, the project addresses existing implementation gaps in coverage, retention, and quality of ambulatory NCD care in India.

Key Objectives:

  • Develop a model to strengthen ambulatory care for NCDs, focusing on coverage, quality, and equity.
  • The study will be conducted in four states—Haryana, Tripura, Maharashtra, and Andhra Pradesh—each with one district selected for implementation.
  • Employ provisional implementation strategies, including governance strengthening, patient empowerment, supervision, evidence-based protocols, and technology utilization.

PROJECT EXECUTION

  • Formative Phase: Understand the context and current implementation status of the NP-NCD program in study districts.
  • Co-design Phase: Engage stakeholders, including patients, implementers, and policymakers, in shaping the implementation model. Develop and refine treatment protocols.
  • Implementation Phase: Pilot and iterate the model, monitor processes, and evaluate outcomes using mixed methods and interrupted time series design.

Duration: Three years, with each phase strategically planned.

 Evaluation Methods:

  • A mixed methods approach includes quantitative and qualitative data collection.
  • Interrupted time series design for concurrent evaluation.
  • Costing analysis to assess additional resources required for scale-up.

Findings will be disseminated through policy briefs, meetings with policymakers, media briefs, stakeholder, and community engagements.

IMPACT

The project has set out to enhance NCD care delivery, reduce out-of-pocket expenses, and inform policy for sustainable scaling across India. This project aims to comprehensively tackle the increasing burden of NCDs in India by strengthening ambulatory care services through evidence-based interventions and stakeholder engagement.
