Integrated-Tracking, Referral, Electronic decision support, and Care coordination (I-TREC) model of care
The Challenge
India’s health system is facing a significant challenge with rising hypertension and diabetes cases. CCDC supported the improvement of the Government of India’s (GoI) National Program for Non-Communicable Diseases (NP-NCD) initiative to address the growing burden of hypertension and diabetes in India. We integrated Clinical Decision Support Systems (CDSS) technology a task-shifting strategy to boost the program’s impact.
